Care and Support planning is about effectively supporting patients with multi-morbidity and the teams that work with them. By 2035 approximately 17% of the UK population is projected to have 4 or more chronic conditions. (1) Locally in Berkshire West more than a third of the population have a Long-Term health condition. The NHS 10-year plan suggests that personalised care and support planning will be part of day-to-day in 2023/24.
